I was thinking this kind of rule only caused a penalty of 9 something something(no loss), but I'm not sure what would be best.

In that case, I think the ruling is as it should be. 3-0 win for death with no extra penalty.

NO one did. It was decided during deck building that Omega or Terro should sub.

I'm pretty sure it was an honest mistake by eam earth (a sad one), but if they did loss that game, there would be no consequence at all for em?  I'm just thinking this is like making a suicide deck without saying so which would be a minor penalty (-3).  This is mostly for the principle of making sure people stay aware of rules all the time and also aren't tempted to cheat.   Imagine if afda had 2 earth nymphs but the team had 2 codes ready to give "in case" they won and then get caught...  this is not a thing we want.
