I noticed a change in card description, not mentioned above:

"The target creature attacks multiple times per turn. Weaker (low atk) creatures gain more extra attacks.

The reason is the misunderstandable meaning of "small" in the old text: "Smaller creatures gain more extra attacks". Otyughs can eat "smaller" creatures.

I also believe that Catapult getting introduced ultimately caused this change; people were confused enough with the damage equation already so it made good sense to finally address this minor error.

In addition to Kuross' idea, I'd say the yellow portion of each player's HP should incorporate any Adrenaline effects.  The two put together would eliminate the arcane nature of Adrenaline for newbie players, and make it more readable and convenient for the pros.

Doesn't Adrenaline fizzle once a creature's attack reaches 16?  The wiki table stops there, but the second attack listed is still 5, so maybe there's more that hasn't been listed.

There seems to be a common misconception among players that a creature with the ability Venom or a Devourer/Pest's passive ability is activated every other "turn" while Adrenaline'd.  In truth, this is incorrect.  These abilities instead activate on the first and last "turns" that the creature executes during that turn.  To test this fact, you can do the following:
1)  Play a Devourer and use Aflatoxin and Adrenaline on it.  The Devourer will absorb quanta that turn and die that same turn.
2)  Infect a Pest and then use Adrenaline on it.  The Pest will absorb quanta once that turn and then die before another quanta can be absorbed.

The first step proves that a Devourer/Pest's ability is activated prior to death and also that quanta is absorbed during the first "turn".  The second step proves that the ability is activated a second time on the fifth "turn" with the foreknowledge that 0 attack creatures receive 5 attacks with Adrenaline.  Because the Pest does not absorb quanta a second time when it dies on the 4th "turn", you can therefore conclude that the second absorption takes place on the fifth "turn".
The exact same applies to Scorpions, etc. as well.

However, if this is the case then why don't creatures such as Scorpions poison the opponent twice under the influence of cards such as Procrastination?  The best way to explain this is to take a step back and reconsider how exactly Adrenaline causes a creature to function.  I keep referring to Adrenaline's effects as "turns", but obviously you're not getting another turn when the creature attacks.  If you interpret the Scorpion's "turns" as attacks then the fact that a Scorpion does not poison twice when it runs into Procrastination might seem quite odd since the Scorpion only gets to attack twice, therefore it should inflict poison on the first and last attacks, right?  Well that's not the case either.  Try thinking about those extra "turns" as actions instead of attacks.  In this case, the scenario would work as follows for the Scorpion:

[Venom + Attack] --> Delay --> Attack --> Delay

When it normally functions as:

[Venom + Attack] --> Attack -->  Attack -->  [Venom + Attack]

Because Delay overwrites the fourth action, Venom is not applied twice during that turn.  By considering the fact that Adrenaline assigns a preset number of actions each turn, this makes even more sense:  Venom may only be applied during the first and fourth actions; if an action is overwritten by a status effect (delay, freeze) or if the creature dies before that final action takes place then this does not change how venom is applied.  So if the Scorpion began its turn Frozen then its action list would become:

Thaw --> Thaw --> Thaw --> [Venom + Attack]

And if a Scorpion was frozen the turn before and just now received Adrenaline then it would become:

Thaw --> Thaw --> Attack --> [Venom + Attack]

And finally back to our Pest example, the normal action list is:

[Devour quanta + Attack] --> Attack --> Attack --> Attack --> [Devour quanta + Attack]

However when it begins the turn infected then it dies before the fifth action, therefore it becomes:

[Devour quanta + Attack] --> Attack --> Attack --> Attack (Dies) --> [Devour quanta + Attack]

See?  When the attack actions are able to be executed doesn't change how the effect is applied.

I've seen a lot of people asking recently about the effects of Dive + Adrenaline:  How are the attacks determined?  Well it's actually pretty simple, but you need to remember 2 key points:

1)  Only the first attack receives bonus damage due to Dive.
2)  The number of extra attacks granted by Adrenaline is determined by the original attack value, not the Dive'd attack value.

So to put this in perspective:
You have a Wyrm on the field.  You Dive it, increasing its attack to 6 and then play Adrenaline on it.  The Wyrm then proceeds to deal 6+3+3+3 damage for a total of 15 damage.
Similarly, if the Wyrm were upgraded in this scenario then it would deal 10+4+2 damage.

Therefore:  Dive doubles the first attack but then allows all other attacks to continue as normal.
