This card seems like an OP darkness phoenix to me and witch should be weakened by possibly reducing attack on witch.

It says Ghost turns into a witch, when does this happen? when it attacks? when it dies?

On Elite Ghost it says
'Elite Ghost turns into Elite Ghost.'

Interesting concept on fusion allowing you generate strong monster somewhat quickly. If anything you should have 1 fusion monster of each element. You could create a card that defuses fusion monsters into their component parts creating a stall for 1-3 turns and allowing your opponent to destroy them and possibly rid you of your fusion monsters.

This card could be used in many situations to destroy or strengthen cards so this card could be useful ,though I would say increase the cost slightly.
