Does this mean that the total account card cap has been raised to 200-something times 999? Or is the cap still 3999 but you could use that 3999 by having 999 copies of four different cards if you wanted to?

It means that - at least on the database side - you can have about 200,000 cards at it will still save it all as 1.4kb.
That does not mean that your browser will like that and it might eventually start sending you error messages about memory or script problems.

Let's say I use Archangel/Guardian Angel's Heal on a Ray of Light. Then I use Fractal on that same Ray of Light. Now, would all those copies get 1/5 or just the original 1/1?

Cards played from the hand are exactly the same no matter where they came from. All the copies would be 1|1.
In this particular case, it doesn't matter anyway because Heal won't increase a creature's HP if it's at full health already, so the original RoL would also be 1|1.

A better example would be using blessing on a ray of light.  (or antimatter or adrenaline or momentum or anything else that changes it)  The rays you fractal and get in your hand will have the original stats.
