Any size works, as long as it's square (or close to square). The image maker will resize it to fit the frame, and it will distort if not square.

It really doesn't, as long as the art is really close to square it should be fine. If art looks good at 265, it will look good at 256.

From Request Card Art "Read this First" post:

"Very simply put; if it's not in the forge or higher, the chances of it ever being used are very, very slim. (In fact, I personally don't bother with cards that aren't in the Armory.) Until it's in there, just a little MSPaint picture you made yourself should be fine. I know it probably won't be much of a looker, but hey, it's about the idea up until there, the Armory is for the polishing."

So, don't request art, and if you're art is crappy don't bother? Seems kinda contradictory to me  :P

Many just strip art from sxc.hu because it's royalty-free.  Although you get people like vrt and you can't help but sit in awe...
