It's not really a big concern but just one thing - in the Permanent category where you could select either weapon or shield, there should also another option for general permanent or a checkbox to disable that. If I've accidentally clicked weapon or shield, I have to reload the form to enter a general permanent in the database. Also, maybe the header should be called "Permanent type". Oh, and how about pillar? (though I doubt there'll be any pillar-like card ideas)

I don't think that adding that option is necessary. If a person clicks Weapon or Shield by mistake (something that probably happens once for every 500 card submissions), he or she can just refresh the form and start again, because up to that point, they have only spent about 5 seconds on it.
I don't think that the Pillar option is needed either. Why cannot they just be regular permanents? Besides, I doubt Elements will see any more Pillars. If there is more need for quanta production, Zanz will most likely use something new, like he did with Pendulums.
Looks good, I agree on most of the categories, but you might want to specify some of them (for example, the differences between direct and indirect), and, maybe add a lost categorie saying other, because I don't see a real place for silence right now.
I'd like to not have "other" option because then people might start using it every time a card is not (in their opinion) a perfect match to some mechanic, leading to a situation where we have tons of "other" mechanics. I think when a card comes whose mechanics we don't have, we should just add a new mechanic to the form.
"Direct Damage" might be better as "Instant Damage", making the other one "Delayed Damage".
For Silence I added "Affect the ability to play cards". Probably not the best one there is, so I'm open for other suggestions.
