NOTES:
I know that people often curse the RNG when their dusk mantle fails or nothing ever seems to go your way, so I thought, why not change that? Presenting entropy's answer for bad luck... a second chance.
-Multiple copies will not stack, just provide some protection against PC
-If both players have a copy, then the effects cancel (no repeated attempts)
-Thematically, santuary cancels this card's effects
-Examples of chance based effects include:
 -shields
  -skull shield: if enemy creature doesn't die to unholy, retry occurs
                 if your creature would die to unholy, retry occurs
  -thorn carapace: if enemy creature doesn't get infected, retry
                   if your creature would get infected, retry
  -ice shield: if enemy creature doesn't freeze, retry
               if your creature would freeze, retry
  -fog shield: if enemy creature would hit, retry
               if your creature would miss, retry
  -dusk mantle: if enemy creature would hit, retry
                if your creature would miss, retry
 -Other cards:
  -chaos seed: if effect doesn't kill enemy creature, retry
               if effect would kill your creature, retry
  -mutation (card and elf's ability): if enemy creature doesn't die, retry
                                      if your creature would die, retry
  -pandemonium: if effect doesn't kill enemy creature, retry
                if effect would kill your creature, retry

Seems well thought-out. The problem is that this card would have to be updated each time a new chance card comes out.

I suppose, I have no experience in how the cards are coded, but could the dice card create a second check loop that is activated whenever a RNG calculates a card effect, and then only use this secondary loop under the conditions like creature death, or status conditions? Any suggestions on how this card could be less complex coding wise?

That can be easily fixed.  Just tack on a marker to each effect code, and then the dice will read the effect marker and decide if the effect needed to be redone or not.
Rule #1: The Captain is never wrong.  Rule #2: If the Captain is wrong, refer to Rule #1.

In any case, whether it's difficult to code or not shouldn't affect whether it's a cool card or not.

It's a simple mechanic with concrete and well thought-out interactions with other cards that can be extended to future cards. +1

Cool, is there anything that I should do for it to be considered for moving up in the Crucible? (This is my first major idea that has been this well received, and that members have actually said could be plausible.) Maybe add a poll?

You did not provide a source for the art for the curator to verify whether it's copyrighted or open-source (and if the artist has requested permission before use, you should say somewhere that you have received permission). Alternatively, you can use a stock image, for example http://www.sxc.hu/photo/1256359 (remember to include the source in the "Art" section of the table).

Now, formatting. You should change the forward slash character "/" in the topic title (edit the topic in your opening post) to a pipe character "|". I would move your opener/introcution into the "Notes" section of the table. Finally, the un-upgraded card, in both the card image and table, seems to have an extra space in its name "Lop-Sided  Dice".

Everything else looks good.
