Confirmed in trainer. I noticed that when a voodoo doll was accelerated (but not purified), the ai health bar would show the -1 from acceleration's health loss but not the damage the voodoo doll did (the health total went down as appropriate, it just was hard to see what was going on). This is a clear graphical bug. Once purified, the ai only lost health from the voodoo doll's attack, neither gaining nor losing health to the combo. I'm not sure whether or not this part is intentional. When destroyed by paradox (Congeal + Shockwave also tested and works (EDIT: also GP + creature damage)), the ai gained 2 poison counter as Tetti stated. To clarify, these were purple poison counters that dealt damage, rather then blue purify counters that heal. Playing multiple purifies on a single doll would increase the poison delivered when destroyed. This is a clear bug, as voodoo dolls are only supposed to do anything if they survive an attack (even congeal+shockwave will not deal the 4 damage to the opponent, or destroy their weapon if the voodoo doll is destroyed).
I assume this happens because the purify counters are removed when the voodoo doll dies, and removing purify counters = adding poison counters. Some of the voodoo doll interactions are counterintuitive, so I won't say that part shouldn't be how it works, but the part about voodoo doll saying "If this creature survives an attack..." still stands.
