Crazy explanation :
Player, Nom, and Wyrm roam ETG.
Player and Wyrm decide to play versus each other using pvp duel. Wyrm thinks Player's in game name is Nom, so he searches for Nom. But Player knows Wyrm's ign is Wyrm, so Player searches for Wyrm.

Result : they connect. (tested)

Why is this bad?
Imagine Player and Wyrm, knowing each other's ign this time, are going to play a duel. They search for each other, but Nom being a troll, he decides to search for Wyrm. And he might connect to Wyrm and stop Player from playing Wyrm. (tested)

I can confirm that it is possible for someone searching in PvP Duel to match up with someone other than who he named.
I can also confirm the obvious complement: that someone searching in PvP Duel can search and match up with someone, even if they are not mutually searching each other.

I believe that it is possible for Person A to match up with Person B if Person A was searching for Person C and Person B was searching for Person D... but I haven't tried it before.

I can confirm this happening a while ago, when I was gonna play two different people, and the second in line thought he was first, and searched and got me, even though I searched for the first person.

It appears that when you search for someone in pvp duel, regardless of whom you search for, you'll get connected with someone that was searching for you, if such a person is registered in the pvp servers (even shortly after that someone stop searching). This is done before any game is created where you search for your opponent.

I always thought of it as a feature: at least one person has to spell the others' name correctly if you're both regularly retrying. Though it can be annoying if a third party is aware of two people going to duel and trolls by entering their name, too.
