Is this stating that EP must have only 1 Ash on her side of the field, or that Ash is the only type of creature that can be on her side of the field?

Only Ash creatures can be on her field (i.e. - if she has anything but Ash, it won't be complete). No Phoenixes (sorry Poofster). ^^;

Does mutating a creature count as "otherwise removed"? And does killing all creatures in her deck mean we have to have to leave her with 0 cards left?

First question: Yes, BUT that creature must also be removed from the field (if you can reverse time it into her deck and she can't play it, it's considered a 'dead' card).

You do not have to kill all of her creatures in her deck, but you DO need to make sure that her field only contains either:
a) Immaterial creatures
b) 4 copies of one of your creatures
