How many times a year does cards go from crucible to forge and from forge to armory?

The crucible cycles through the element order in the bazaar and other, sending 1 card from 3 or 4 different elements to the forge each week. (counting other as an element in this case)

The forge is divided into card type. 1 card from each type is sent to the armory each month.
