Upped is now quite a bit stronger than the unupped. Not sure if OP though, perhaps the unupped is UP.
What about changing the unupgraded stats to 1|8? Then the unupped wouldn't be UP and it would be more clear whether the unupped was UP or the upped was OP. Moreover, I noticed that the Doom Tank can't use it's skill on the first turn, so with 6 HP it would only cast twice its ability.
Don't forget, however, that purify can always be used to remove the poison counters.
If you make the other advantages potent enough this will essentially just become a forced combo.

So in a sense, the most cost benefit you can gain out of having poison counters is 1 to 2 :underworld + 1 extra card advantage (~2 :underworld value... though this is sometimes debated)

So in all, a starting poison count is worth at most 3 :underworld upped and 4 :underworld unupped.

So going to total quanta value of the card, you get:
unupped: 6 :underworld (cost) + 4 :underworld (poison count) = 10 :underworld for a 1|8 with the doom ability
upped: 6 :underworld (cost) + 2 :underworld (upgrade) + 3 :underworld (poison count) = 11 :underworld for a 1|6 with doom and voodoo

I'll leave the assigning of cost-theory value to att|hp and the doom and voodoo abilities to others, but this should be a good starting point:

It's a bit confusing why you used :underworld quantum for description instead of :death. It's already confusing enough without that anyway. I read through what you wrote 5 times in a row, but it still doesn't make much sense to me.

I made 3 versions with different kind of quanta-HP redistributions. Which one do you like better? Other options are welcome, here is the table code:

A random idea. How about removing the poison and change the ability to:

 :death :death :death :death: Doom.
Doom tank and target creature suffers 15 damage after 3 turns.

So it'll be a kamikaze weapon, which spread Deathness till it's dies. The upped could be OP though, but I think not.

That would be viable, but if I go with that, I should both drop the poison and the voodoo. Maybe that would be the right choice as skyironsword also mentioned earlier, but I'm a bit hesitant on dropping both. But if you both agree, I will change the card to the new ability what Zergva said. However, Chapuz also mentioned that it can go pretty OP if exploited with BB and PU, so that is also kept in mind.

I don't really see the quanta optimalised version of the tank-BB-PU deck. It's need too much wait from your opponent, no CC-s for 3 turns, no attack or no PC for the Dim. shield spam (which needs more  :aether quanta. And this not that type of trio, what can be changed to SN based. And still an RT can ruin everything. Although my idea was a random suggestion, so if it'll become popularly favorited, it can, but if not, just forget it :D
