Being a Discworld fan, I'm certainly not one to say that a chest should not be a creature.

That said...maybe some legs drawn on the upped?

Is it the Open ability that has 3 uses or the permanent generated from it that has 3 uses?

It's the Box/Chest itself that has three uses.

Creature chests are too common nowadays. Regular chests are a dying race.

If you were dead set on making it a creature, mimcs are often portrayed as treasure chests.  (before they grow tentacles and eat your face off!)

Would the unupgraded/upgraded status of the random card depend on the status of this card?  (thinking along the lines of how pharaohs spit out scarab or elite scarabs based on if they are upgraded or not)

Yes. Forgotten Chest would spit out upgraded permanents.

Very interesting. I would like to see a card like this in use. I think that it would be alright to allow it to give shields and weapons. Perhaps only if you don't have any already.
