Phoenix Not Turning to Ash

Twice in one game I got a phoenix that didn't turn to ash when it died.  I was left with nothing.  The first time, I Immolated the phoenix, which has created ash in the past.  The second time, a phoenix was destroyed by Maxwell's Demon and left nothing when I think it should have left ash.

A third phoenix was destroyed in the game.  I forget how it happened, but it left ash that time.

If you were playing against a Maxwell's Demon then you likely had your Phoenix Lobotmised by the AI's Chaos Seed card. The ability allowing Phoenix to return to Ash is a Skill and if that skill is lost the Phoenix will die without leaving Ash.

is phoenix/ash supposed to be immune to aflatoxin/death nymph? when you put the cancer poison on phoenix, it dies without creating a cancer cell, and it turns into ash as normal. if ash has a cancer poison, it loses the token when it turns back into phoenix.

that sounds like a wacky mechanic to fix, but it doesn't seem to be functioning as designed.
