Try to recreate the scenario and pay close attention to the border of your Otyugh. When it attacks against Fire Shield, its border will flash red for a second and its health will decrease, unless you made it immortal or put momentum on it. I haven't had this bug happen to me lately, so I can't confirm this myself.
